Thank you for registering with St. Mary & St. Joseph’s Catholic Community. Filling out this online form will officially register you and your family with the parish. This registration form will take between 5 and 45 minutes to complete, depending on the size of your family. There is no facility to save your answers and come back, so be prepared to complete the form in one session. This form collects information about you and your family, including detailed information about your family’s children and sacraments.

Information requested includes:

  • spouse’s information
  • number and ages of children including birth dates
  • sacramental dates for all family members including;
    • baptisms
